BAKER ELEMENTARY SCHOOL: Back-to-School Physicals, Dental Exams, Immunizations & Sports Physicals

Baker Elementary School issued the following announcement on Jul. 13.

Important information for families with new students and those entering grades 3, 6, 7 and 11 as well as our Fort Couch and USC High School student-athletes. Please make sure your child’s health information is up-to-date.

Physical Exams: Kindergarten, Grade 6 and Grade 11

The Pennsylvania school code mandates medical examinations for students entering kindergarten, grade 6 and grade 11 because these grades represent important periods of growth and development in a student’s life. The major objective of these examinations is to ensure that the student is in a general state of good health.

This requirement may be met by either a physical done by the student’s private physician or by the school doctor. If a private physician is preferred, please have your physician compete and return the Private Physician Report of a Physical Examination form.

Students who do not return a private physician form will be scheduled to see the school physician in the spring with parental permission.

Dental Exams: Kindergarten, Grade 3 and Grade 7

The Pennsylvania school code mandates dental examinations for students entering kindergarten, grade 3 and grade 7.

This requirement may be met by an examination by the student’s private dentist or by the school dentist. If a private dentist is preferred, please have your dentist compete and return the Private Dental Examination Report form. Students who do not return a private dentist form will be scheduled to see the school dentist in the spring with parental permission.

2021-22 School Vaccinations

The school nurse maintains an official Pennsylvania immunization record for each student. When your child receives additional immunizations, please send information to the school nurse to ensure that your child’s record is up-to-date.

In accordance with the Allegheny County Health Department, the immunization guidelines for students are provided below.

All Grades K-12

4 doses of tetanus, diphtheria, and acellular pertussis* (1 dose on or after the 4th birthday)

4 doses of polio (4th dose on or after 4th birthday and at least 6 months after previous dose given)**

2 doses of measles, mumps, rubella***

3 doses of hepatitis B

2 doses of varicella (chickenpox) or evidence of immunity  

For attendance in 7th Grade

1 dose of tetanus, diphtheria, acellular pertussis (Tdap) on the first day of 7th grade.

1 dose of meningococcal conjugate vaccine (MCV) on the first day of 7th grade.

For attendance in 12th Grade

1 dose of MCV on the first day of 12th grade. If one dose was given at 16 years of age or older, that shall count as the 12th grade dose.

Sports Physicals/Online Athletics Registration

In order to participate in fall sports at Fort Couch Middle School or Upper St. Clair High School, student-athletes must upload their completed sports physical form to Healthy Roster, the district's online athletics registration portal, prior to the start of the first fall practice. Sports physicals must be completed June 1, 2021, or after and are valid for one year.
